I don't have access to that file, however good information and examples for creating custom input controls can be found in Chapter 4 of the Administration Guide:

I have attached an example from 4.7 to the original question.  I don't know if it will work for the current version.

Is there any way you can edit the title of this post to specifically mention input controls so that people can find it in searches?  For example, "can someone post this input control attachment?"
