extracting results from a mysql DB in a template

Hi,

I want to extract mysql DB results in a  form -template with a specific format having checkboxes etc; Is it possible with ireports, please advice.

 

For Eg.

Lastname-------   Firstname----------

Education

chkbox  Graduate

chkbox  undergraduate

chkbox   Associate Degree

 

... etc in this format. How doI create a template with checkboxes and how do I get the results in this template from the databasr, I am using PHPMyadmin.

The way it use to be done would be to include a check box image in your report. If you want to show a box checked or un checked you can use a expression to control which box you display in your field based on a condition.
